Why do nonprofits need an IRS Form W9 (W9 Form) ?

The IRS requires our organization to have W-9 forms on file.  A W-9 form establishes your identity (individuals) or the identity of your organization.  Simply put, in order to maintain accurate bookkeeping and stay in compliance of IRS mandates, we must receive a complete W9 form (from you) before issuing a check.  

 

But I don't know how to fill out a W9 Form!  

We have your back.  Below are a few examples of how to fill out your form.  Here's a few things to remember when filling it out: 

1.  If you applied for this grant as an individual (with your name) - then you should fill out the W9 form with your personal information only.  That is, list your first and last name as it is listed on your social security card.  You will also list your mailing address and your social security number.  Your grant check will be made out to you and you will be personally responsible for paying taxes on the grant.  The check will be mailed to the address listed on the W9 Form.  Here's an example of what your filled out form should look like: IRS W9 FORM - Individual .

 

2.  If you applied for this grant as an individual with a nonprofit serving as your fiscal agent- then the W9 should include the nonprofits information only.  That is, the nonprofits business name, address and Tax ID number should be listen on the W9 form. The grant check will be mailed to the address listed on the W9 Form.  Here's an example of what your filled out form should look like: IRS W9 FORM - Individual with fiscal agent. 

 

3.  If you applied for this grant under a non profit organization/school name or school system- then the W9 form should include the non profit/school/school system's information.  That is, the nonprofits business name, address and Tax ID number should be listed on the W9 form. The grant check will be mailed to the address listed on the W9 Form.  Here's an example what your filled out form should look like: IRS W9 FORM - Nonprofit.

Forms that are outdated or filled out incorrectly will not not be accepted thus delaying the delivery of your grant award.
